Release from Liability/Property Agreement

I, parent/guardian of the above registered children, give my child(ren) permission to accompany their Vacation Bible School group at the VBS campus that is assigned to the child. The volunteers will have a mandatory background check. I understand that while on the property of any of the VBS campuses, my child will be covered by that church's liability insurance if something were to go wrong with the building or equipment that belongs to that building. I further expressly authorize and consent to any medical care by qualified medical personnel for my child, if necessary, because of illness or injury.

All children are welcome to VBS! There event is completely FREE!
If you wouls like to give a donation to the United Methodist VBS program, there is an option for you to do so. If you would cannot or do not wish to donate at this time, please click the "no donation" option. These donations help defray some of our daily costs (craft, rec & science supplies, snacks).
